* You may have more than one entry (i.e., one as an artist and one as a business, etc.) 

* Please limit to 3 attachments. If uploading documents, each page must be

   uploaded separately. 

* Administrators reserve the right to delete and/or correct any information. Entry's

   will be checked periodically and adjusted accordingly.
